Summary
The Advanced Practice Nurse is a licensed independent practitioner with advanced education and national certification who will provide evidence-based care for a population of patients. The Nurse Practitioner will provide a full range of services emphasizing quality - timeliness - and the advancement of the Veteran Affairs (VA) mission.

Duties

  • This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until May or until positions are filled - closed - or canceled

  • Montana: The Advanced Practice Nurse (APN) will practice in the Primary Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
  • The Nurse Practitioner will provide advanced clinical practice - consultation - and management
  • The Nurse Practitioner practices to the full extent of their education - training - and certification
  • Core functions are outlined in the clinical privileges that correspond with this position
  • The Nurse Practitioner prescriptive authority for a panel of patients or program as indicated by their position
  • MAJOR DUTIES: The Nurse Practitioner is a licensed independent practitioner with advanced education and national certification who will provide evidence-based care for a population of patients
  • Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
  • Develop and implement an individualized plan to achieve optimal population health and disease management
  • Demonstrate leadership and work collaboratively with Primary Care Team and other interdisciplinary team members
  • Order and interpret labs - tests - and diagnostic studies
  • Prescribe pharmacologic and non-pharmacologic interventions
  • Initiate referrals and consult to specialty services
  • Provide education to patients and their families
  • Assess and adjust the plan of care as needed to achieve optimal patient outcomes
