Visiting homes to determine health situations and dialogue with household membersIdentifying and sensitizing communities on health interventionsIdentifying common ailments and minor injuries at the community levelCollecting health data at household level for analysisDistributing Information Education Communication materials, mosquito nets and other commodities to householdsIdentifying defaulters of health interventions and other health cases and referring them to health facilitiesMaintaining and updating health data using health registers
